You can use this form to submit a Vote-by-Mail ballot request online. Requests expire after every General Election (December 31 in even numbered years). Additional information about voting by mail can be found on our Vote-by-Mail Information page.

• If you need to update your residential or mailing address, please do so before you request your ballot. You can update your address online at RegisterToVoteFlorida.gov.
• If you have a protected address or have a disability and would like an accessible Vote-by-Mail ballot, please call or email us to submit your request.
• If you have a new Florida driver’s license or state ID card with a new number, we may have your old number in our system. This may stop you from using this online form. If this happens, please call and we will help you over the phone.
